School physicals are required for students entering Kindergarten,
in the 4th and 7th grades and those who wish to participate
in the sports program. Physicals must be conducted by the
student’s own physician and documentation must be
sent to the school on the
Health Examination Form. Physicals are good for one
year from the date of examination. If a physical expires
during the sport season, it must be renewed before continued
participation will be allowed. Please contact Mrs. Lehmann,
school nurse, via the business office, if you need additional
information regarding these requirements.
